1.아레노바이러스 감염으로 인한 출혈열 Hemorrhagic fevers Caused by Arenoviruses 

원인

아레나바이러스

종류

1. 라사 열(Lassa fever)의 원인이 되는 라사 바이러스(Lassa virus)

2. 아르헨티나 출혈 열(Argentine hemorrhagic fever)을 일으키는 쥬닌 바이러스(Junin virus)

3. 볼리비아 출혈 열(Bolivian hemorrhagic fever)을 일으키는 마쿠포 바이러스(Machupovirus)

4. 베네주엘라 출혈 열(Venezuelan hemorrhagic fever)을 일으키는 구아나리토바이러스(Guanaritovirus)

5. 부라질 출혈 열(Brazilian hemorrhagic fever)을 일으키는 사비아바이러스(Sabia virus) virus)

6. 림프구성 맥락 뇌막염(Lymphocytic choriomeningitis)을 일으키는 림프구성 맥락 뇌막염 바이러스(Lymphocytic choriomeningitis)

매개체

쥐의 오줌 등에 접촉.

증상 징후

아레나바이러스 혈청형의 종류에 따라 생길 수 있는 출혈열의 증상 징후와 병세의 정도가 다르다.

출혈 열을 경도중등도중증 출혈 열로 분류한다.

두통근육통결막하광범위출형출혈복통겨드랑 출혈반점점막 출혈인두염혈관손상혈소판 감소증혈소판 기능장애여러 기관 장애등이 생길 수 있고 전신 출혈이 생길 수 있다.

단백뇨쇼크상기도염과 하기도염뇌염뇌병진전공포증의식 상실혼동경련 등이 급성으로 생길 수 있다.

잠복기는 6~17일이다

진단

병력 증상 징후 진찰소견 등을 종합해서 진단한다.

아레나바이러스 항체 IgM과 항체 IgG, 항원을 검사, IFA 검사, Reverse trascriptase polymerase chain reaction (RT-PCR) 검가로 진단할 수 있다.

치료 예방

아레나바이러스 출혈 열의 치료

회복기 혈장 주사로 치료할 수 있고리바비린(Ribavirin) 항바이러스제 혈관주사 등으로 치료할 수 있다. .

발생지역

남 아메리카중앙 아메리카서 아프리카베네주엘라사비아 지역에서 주로 이 병이 유행하고 미국에서도 가끔 발생된다.

 

2. 버나바이러스 감염으로 인한 출혈 열 Hemorrhagic fevers caused by                        Bunyaviruses-이 바이러스에 감염되면 신체각종 계통에 감염되어 심하게 앓을 수도 있고 죽울 수있다. 다음(1)-(3)과 같은 병이 생길 수 있다.

(1).신증후군을 동반하는 출혈 열 Hemorrhagic fever with renal syndrome

원인

버나바이러스 속의 한타바이러스 감염된 설치류 동물과 절지곤충 매개로

증상 징후

두통복통요통결막출혈저혈압출혈 반점감뇨증무뇨증다뇨증신부전증,

쇼크 등의 증상 징후가 생길 수 있고전신 출혈이 생기고 신장 장애로 사망하는 것이

보통이다.

진단

병력 증상 징후진찰소견 등으로 진단한다.

환부조직 등 피검물로 바이러스를 배양해 진단한다. PCR 분석, EIA 검사, IgM, IgG검사

등으로 진단한다.

치료 예방

지지치료대증치료항 바이러스제 Ribavirin으로 치료하고 투석치료쇼크치료고혈압치료심부전 치료한다.

예방백신은 없다.

발생지역

아시아동서 유롭한국에서도 유행한다.

 

(2) 크리민 콩고 출혈 열 Crimean-Congo Hemorrhagic fever

원인매개체

나이로바이러스(Nairoviruses), 진드기 물림 매개로 감염될 수 있다.

증상 징후

간염광범위 모세혈관 출혈두통근육통결막염출혈반점위장출혈 등의 증상 징후를 동반하는 급성 출혈열의 일종이다.

진단

병력 증상 징후진찰소견 등으로 진단한다.

환부조직 등 피검물로 바이러스를 배양해 진단한다. PCR 분석, EIA 검사, IgM,

IgG검사 등으로 진단한다.

치료 예방

지지치료대증치료항 바이러스제 Ribavirin으로 치료하고 진드기에 물리지 않게 주의한다.

발생 지역

아프리카중동중앙아시아유럽 등에서 유행하고

 

(3) 리프트 계곡 열 Rift Valley fever

원인

리프트 계곡 바이러스(Phleboviruses), Hertland Virus(미국), 모래 열 바이러스(유롭)

중증 열 혈소판감소증 증후군 바이러스(중국

매개체

모기에 의해서 감염될 수 있다공기로도 감염될 수 있다.

증상 징후

대부분의 경우 자기 한정성비특이성 열성 증상 징후를 나타낼 수 있다.

때로는 출혈 열쇼크황달간염뇌염망막염 등이 생길 수 있는

급성 바이러스 감염병이다.

진단

병력 증상 징후진찰소견 등으로 진단한다.

환부조직 등 피검물로 바이러스를 배양해 진단한다. PCR 분석, EIA 검사, IgM,

IgG검사 등으로 진단한다.

치료 예방

대개 일정한 기간이 지나면 자연 회복되기도 하고

지지치료대증치료항 바이러스제 Ribavirin으로 치료한다.

환자와 접촉을 피하고 모기 매개체에 물리지 않게 곤충 퇴치제를 이용한다가축에게 예방접종을 해준다.

발생 지역

아프리카사우디 아라비아 등 지역에서 유행한다.

 

3.필로바이러스 감염으로 인한 출혈열에볼라 출혈 열 Ebola hemorrhagic fever/Marburg hemorrhagic fever

역학 원인

Marburg virus 감염병을 에볼라바이러스(Ebola virus) 감염병이라고도 한다.

에볼라바이러스는 필로바이러스 filoviruses 군에 속 한다.

과일 박쥐가 에볼라 바이러스를 보균한다사람이 박쥐의 변이나 침 등에 노출되면

이 병에 걸릴 수 있다.

이 병을 앓는 사람의 경구나 호흡기를 통해서 나온 바이러스에 노출되거나환자의

점막에 접촉되거나피부 상처 등을 통해서 다른 사람에게 감염될 수 있다.

이 병으로 죽은 시체를 접촉해도 이 병에 감염될 수 있다.

매개체

과일 박쥐.

증상 징후

심한 두통근육통허약피로설사구토복통원인불명 출혈자반증 등의 증상

징후가 생기는 전신 감염병이다이 병에 감염되면 심하게 앓고 25~90%가 사망한다.

소아들이 이 병에 감염되면 호흡기 증후군이 더 많이 생기고 중추신경 증상은 덜

생긴다황반구진 피부발진이 얼굴과 몸통에 생긴다결막충혈결막하 출혈 등이

생긴다간 기능 장애저 포타시움저 나트리움저 칼슌증저마그네슘증도 생긴

.

위장출혈신장부전증 생기고 임신부가 이 병에 걸리면 낙태하고 질 출혈이 생긴다.

그리고 사망률이 거의 90%이다에볼라 감염병에 걸린 산모로부터 출생한 신생아는

거의 다 사망한다잠복기는 2~21일이다

진단

병력 증상 징후 진찰소견외국 여행 경험에볼라 환자에 접촉 병력 등을 종합해

이 병을 의심하면 필로바이러스 감염병(Filovirus disease)은 RT-PCR 분석 검사,

ELISA 검사, IgM, 세포 배양 검사 등으로 진단한다.

말라리아홍역뎅기열장질부사라사열 등 다른 감염병과 감별 진단해야 한다.

치료 예방

포도당 전해질 용액 주사 치료혈압 상승제혈관영양제 공급세균 감염이 있으면

항생제 치료를 한다. ribavirin이나 스테로이드 치료는 치료 효과가 없다.

발생 지역

서 아프리카나이제리아세네갈미국영국콩고 등 지역에서 이 병이 발생했다.
